I am between 18 and 65 years of age
I weigh at least 45 kg (whole blood donation)
I am in good health today — no fever, cold, cough or infection in the last 2 weeks
It has been at least 3 months (men) / 4 months (women) since my last whole blood donation
To the best of my knowledge my haemoglobin is ≥ 12.5 g/dL (I am not anaemic)
I am not on antibiotics, steroids or blood-thinning medication right now
No tattoo, piercing, major surgery, dental extraction or vaccination in the last 6 months
I am not pregnant, breastfeeding, or within 12 months of childbirth (if applicable)
I do not have HIV, Hepatitis B, Hepatitis C, syphilis or any other transfusion-transmissible infection
I have not engaged in high-risk behaviour (IV drug use, unprotected sex with unknown partners) in the last 12 months
The information I am providing is true — I understand giving false information can harm patients

This is a self-declaration. The actual blood bank will re-verify eligibility (weight, BP, haemoglobin, medical history) before drawing blood.
